    • Computer/Smart Phone with Camera– You will be requiredto upload pictures of the documentation below documents during your application session. Please have these documents on hand and ideally already saved as pictures in your computer/smart phone before you begin:
  • Government Picture ID for Parent/Guardian(Unexpired)
  • Proof of Address– You can upload your picture ID again if it has correct information. If not, please have a bill or lease/mortgage document with your name/address as proof.
  • Birth Certificates for ALL Children ages 0-12 – We must be able to verify each child’s age.

*If you are not the parent of the child you are applying for, YOU MUST provide proof of legal guardianship. That document must include your name, the child’s name and their birthdate.

You must upload the documents above when you apply.

Tips for a successful application:

  1. Contact information – We need correct contact information, including an email address and phone # if we need to contact you with any questions. Please also ensure your mailing address is correct as that is where we will ultimately send your appointment postcard in November.
  2. Be accurate – Make sure everything is spelled correctly and all birth dates are entered correctly. Do not skip any sections.
  3. Only apply once – We only need one complete registration for your entire household (with uploaded documents). You’ll know that you’ve accomplished this when you get a confirmation code from the application site. No code = not in the system. Include all 0-12 year old children under your custody in one single family application. Do not apply multiple times or for the same child more than once as the system will see this and flag your account.
  4. Try your smartphone – The application site works with smart phones and makes the documentation upload much easier especially if you have pictures of your documents already in your phone. Using a computer or tablet without a camera will require you to load picture images onto that device before you begin.
  5. Upload your documents – If you do not complete this step, your application will not go through. Please use “Medium” image size if you can. If you get to the upload screen and don’t have what you need or have technical difficulties, save the website (URL) and you’ll be able to return later to finish the upload portion.
  6. Keep track of your Preregistration number – It is the easiest way to find your information. You will only get this if you complete the entire application process with required uploads.
